By now, your claim has been filed, and we’ve coordinated communications between your employer, doctor(s) and you. Here are a few details to keep top of mind:

Your Claim Representative

Keep your claim representative updated on:

  • Changes to work status recommended by your doctor
  • Return-to-work updates
  • Every doctor appointment and any concerns about care

Medical Bill Information

Provide this billing address to your medical provider(s) to avoid being billed directly. If you receive a bill, please either mail a copy to the P.O. Box below or text your claim representative an image of the bill.

HIPAA Information

Complete and return your HIPAA Authorization as soon as possible. Send the signed form to your claim rep by email, text or fax so we can work with your doctor and manage your claim.

Please note that not all claims require a HIPAA authorization form.
